In light of recent changes to the COBRA administration process, GCADA has developed the attached updated Group Health Plan Termination Notice. As you know, this form is to be faxed to GCADA within 7 days of an event. Please begin using this new notice immediately and dispose of any and all copies of the previous notice used.
