Effective immediately, all new applications for health insurance through the GCADA Group Health Plan must include complete Social Security numbers for all parties requesting coverage. In the case of newborn babies without Social Security numbers, the child will be added to the insurance but the GCADA Group Health Department must be notified immediately when a Social Security number is issued. While this information is normally provided to us appropriately on the applications, please remember to check any new applications for missing information before sending to the GCADA Group Health office.
