Compliance With Health Care Reform Changes

The GCADA Group Health Plan is proud to announce Plan Amendments have been finalized and installed to ensure compliance with recent Health Care Reform changes to allow coverage for Adult Children and Plan Lifetime Limits.
As a courtesy, Medical Mutual of Ohio has sent a notice to all current participants. Participants will have until February 1, 2011 to re-enroll if they have previously met the Lifetime Limit or wish to add Adult Children under the age of 26. Coverage on Adult Children will begin January 1, 2011, and will terminate at the end of the month of the Adult Child’s 26th birthday.
The GCADA process for this special enrollment is:
1. Complete and fax a new application for “Policy Change” with the new enrollee’s name, date of birth, and social security number to: 440 922 0160 before 2/1/11. Please mail the original application to GCADA.
2. January and February billings will reflect these changes with corresponding premiums.
3. Medical Mutual of Ohio will issue insurance cards to the home of the policy holder in the usual manner.
Policy Change forms for this special enrollment period received on 2/1/11 or after will be subject to the six month waiting period. Each employee must complete a new policy change form to add their Adult Child(ren) effective 1/1/11, even if they may have noted the dependent(s) on an application in the past.
